Heaven and Hell es el nombre dado al grupo formado por Tony Iommi y Geezer Butler, integrantes de Black Sabbath, con los antiguos miembros de la misma banda Ronnie James Dio y Vinny Appice. Los cuatro grabaron y fueron de gira juntos como Black Sabbath entre 1980 y 1982, y otra vez entre los años 1991 y 1992. En 2006 se reunieron de nuevo para grabar pistas adicionales al álbum recopilatorio Black Sabbath: The Dio Years, que sería editado en breve. Fue entonces cuando decidieron hacer una gira juntos entre 2007 y 2008, tocando los temas que grabaron cuando estaban en Black Sabbath, aprovechando que el vocalista original y actual de Black Sabbath, Ozzy Osbourne, estaría ocupado girando con su la banda que formó en solitario.
A causa de la continuidad de la banda con su formación original (Tony Iommi, Geezer Butler, Ozzy Osbourne y Bill Ward), y que el mismo año 2006 fueron colocados en el Salón de la Fama del Rock and Roll, Tony Iommi y Sharon Osbourne firmaron un acuerdo por el cual el nombre de Black Sabbath no se podía utilizar a no ser que Ozzy fuera uno de los miembros, así se hizo bajo el nombre «Heaven and Hell». El nombre proviene del primer álbum que grabaron los cuatro juntos, tras la entrada de Dio en Sabbath en 1980.
El 27 de octubre de 2007, tanto ‘www.black-sabbath.com’ como ‘www.blabbermouth.net’ informaron que los miembros habían decidido seguir trabajando juntos, y grabar y sacar al mercado un nuevo álbum en 2008. Dicha información fue confirmada por el mismo Ronnie James Dio en una entrevista publicada en la web Komodo Rock el uno de noviembre de 2007
